Release Form

I hereby authorize American Dance Academy, its agents and/or employees to obtain treatment for my child for any injury or illness that he/she may suffer while attending classes conducted by ADA.  I hereby release ADA, its agents and/or employees from liability of any kind or nature, excepting only liability based upon gross negligence, relative to actions or decisions that concern or affect my child.
